The private coverage category includes plans obtained through an employer, purchased directly, or purchased through the Health Insurance Marketplace or a state-
based exchange. The category excludes plans that paid for only one type of specialized service, such as accidents or dental care. Private health insurance includes
managed care such as health maintenance organizations (HMOs).

The Medicaid coverage category includes children who had Medicaid or other state-sponsored health plans, including the Children's Health Insurance Program (CHIP).

Children not covered by private insurance, Medicaid, CHIP, state-sponsored or other government-sponsored health plans (starting in 1997), Medicare, or military plans
are considered to have no health insurance coverage. Children with only Indian Health Service coverage are considered to have no health insurance coverage.
